I gather by your Icon you have a 76???  If so Speedway has a rotor that is a five lug replacement for your setup.  New not redrilled and does not require changing anything else.  sorry of course this is for the front.

I just did the front 5 lug swap with speedway rotors...took all of an hour

I did the Speedway conversion on the front, and got 5-lug 28 spline rear axles from Moser.  They included retaining plates from an 8.8 rear that fit fine, and used early 90's Ranger brake drums.  Fits like a dream.

Rotors, Maverick
Calipers, GM Metric (off of a Monte Carlo, Cutlass etc)
Caliper Brackets, rjays.com
I've got less then $100 in my parts for my Front end swap!
