The cam has a disk on the back side of the gear to keep the belt forward, the crank gear has the disk on the front to keep the belt back. If the disk was left off the crank gear, or if it was installed wrong, the belt could move forward (and be riding on the edge of the pulley).

My question is: Is the belt hanging off the pulley or just not centered?

yes you can fix it real easy find the round steel backing plate that came on it stock.
may be the guy who re did it forgot what it was and hid it or trashed it.

replace it no more riding foward
