power brake swap

Hello, I was wondering what all is involved to change over from manual brakes to power brakes?  I know I'll need the power booster and master cylinder.  I imagine there is a spot for the vacuum to attach for it?  I know I will also need the pedal assembly and the mounting bracket the power booster attaches to.  Where can I get a distribution block/proportioning valve for the setup or do I have to go after market?  How do I know if I am getting the right size outlets?  Do I have to change any of the brake lines?  Any thoughts or experiences on this would be greatly appreciated.  Thanks!
